LifeCare Advisors takes the uncertainty out of growing old by customizing a LifeCare Plan that blends the client’s wishes related to healthcare, financial, and housing with recommendations and resources (financial, community, and educational) identified by our experienced professionals.

The objective of a life care plans is to ensure that each client has a road map for addressing aging-related decisions --a plan that can be implemented in stages to maximize quality of life and the preservation of wealth… now and in the future.

With a focus on quality of life, it is no surprise that the planning process itself, in advance of a crisis or problem, is beneficial to the client and family. By thinking through the “what ifs” with objective and experienced professionals to guide the process, the emotion and stress often associated with the discussion of aging-related choices or concerns is greatly reduced or eliminated.

Additionally, clients and their families have peace of mind knowing that there will be little, if any, crisis-mode decision making regarding health, financial, and housing choices when the client’s current situation suddenly changes. The plan will just need to be implemented. And, LifeCare Advisors is available to assist with that as well.

The professionals at LifeCare Advisors remind clients and their families that a life-care plan is a dynamic plan, in that it can be updated and changed as the client’s life situation changes—a spouse becomes ill, an adult-child caregiver moves, etc.—and as new and appropriate resources or options in the community become available.

The Plan provides the client and appropriate family member with a step-by-step guide to follow in addressing expected and unexpected changes as the client ages.

For that reason, a fundamental aspect of LifeCare Advisors’ service is maintaining an ongoing relationship with clients and/or key family members to ensure that the communication lines stay open and that the plan is updated as appropriate. Additionally, LifeCare Advisors is available to assist with the implementation of the plan — serving as the client’s advocate and as an ongoing resource for the client and family members.
 

Steps in the Life Care Planning Process

  1. Preliminary Planning Conference to identify issues and wishes.
  2. Initial Health Assessment—the nurse/care coordinator works with the client and family to determine the current status of a client’s health, medications, ongoing medical issues, and current living situation (housing, family support, etc.) and preferences for the future.
  3. Development of LifeCare Plan by LifeCare Advisors.
  4. Plan Review with the client and appropriate family members, including any legal documents (wills, trusts, advance directives, powers of attorney, etc., as appropriate).
  5. Plan revisions, if necessary.
  6. Plan Completion and Presentation to client and family members.
  7. Plan Implementation.
  8. Ongoing Communication, Evaluation, and Updates, as appropriate.
